Brock Family Care is a Direct Primary Care (DPC) practice model. This model allows us to provide a more personalized and affordable healthcare without the influence of insurance companies and government agencies.
Prepaid Monthly Memberships
There are two membership types. Please see below the details for each of them.
As of January 1, 2023, there will be a one-time $50 registration fee for new members.

Clear, Up-Front Cost. No co-pays. Our members pay a flat monthly fee for access to the below services.
- Unlimited Visits, including telehealth visits during business hours (After-hours or weekend urgent visits will be $50.)
- 2 sets of complete lab work per year per person:
- CBC w/ diff
- CMP
- Hemoglobin A1C
- Ferritin
- Lipid Panel
- TSH+Free T4

- PSA (for men after 55)
- 1 annual physical exam including an EKG and Urinalysis
- House calls as appropriate - up to 2 free visits a year. (After the second home visit, there will be a charge of $50).

We offer physical exams for non-members for a flat fee, as seen below:
Annual Wellness PE (Female w/ PAP) - $250
Annual Wellness PE (Female w/o PAP) - $200
Annual Wellness PE (Male) - $200
DOT/CDL PE - $100
**For new patients, there will be a $25 surcharge for physical exams.
Urgent Care Visits
We offer urgent care visits for non-members as seen below:
$200 fee per visit (any tests or procedures done in-office will be charged at an additional fee)
$50 additional charge if outside of business hours
Any specialty labs are also charged at a wholesale rate and the price will be given to the patient at the time of visit.
Cancellation and No-Show Policy:
If patients fail to cancel their appointment at least 24 hrs in advance or no-show, they will be charged a $50 fee.
If they have a payment source on file, we will use that payment source to collect the $50.
New patients are required a $50 deposit when booking the first appointment, which will be applied towards the first visit or first month of membership. If the new patient no shows or fails to cancel their appointment within 24 hrs, the fee will not be reimbursed.
We do NOT file any kind of insurance for the services we provide, and patients are expected to pay upfront at the time of the visit unless enrolled in our membership plans. We do not send bills after your visit. However, we can send your insurance information with your lab order to LabCorp for them to file your insurance for the labs we draw in-house and send off for processing. You may choose to file your own commercial insurance for reimbursement of our services, but they may pay you back as out-of-network coverage since we are not contracted with any insurance. Therefore, if you pay $125, depending on your plan, you may only get reimbursed $20, $5, or nothing at all. Also, you may not file for reimbursement if you have Medicare or Medicaid.
